I authorize Fizogen Precision Technologies to automatically charge my primary credit card $229.90 every (2) two months. I understand that I will receive an email after each transaction to alert me that my credit card has been charged. I also give Fizogen the permission to notify me via this email address of any future Fizogen Precision Technologies special promotions. Charges will appear on my credit card statement as Fizogen Precision Technologies. Fizogen reserves the right to terminate this payment plan without prior notice at any time . I also understand that at any time, I may choose to discontinue my enrollment for "The Continuous Cycle” plan by providing written notice (email is accepted) to Fizogen Precision Technologies. Unless discontinued by myself, I understand that this authorization is valid until the termination of this agreement with Fizogen Precision Technologies or until my credit card expires.
